They inform you of the circumstances in which you participate or your child participates in the any program of the Bukit Timah Juniors Pte Ltd (known as Bukit Timah Juniors Football Club or BTJ), including training sessions, training scrimmages, league and tournament matches, and other events, at your own risk. Please read all of the information carefully, fill in all blanks and check each paragraph where indicated before electronically signing at the bottom. You should read the statement and understand the information provided, checking the box to signify that you have read and understood the statement. If you do not understand the information provided in this statement you must discuss with us and seek further clarification. If the participant in BTJ is a minor under the legal age of majority (which is 21 years of age) the check mark and electronic signature of the participant’s parent or guardian must be obtained in order for the participant to participate in BTJ activities. In this statement “you” means any participant in BTJ who is over the legal age of majority or any parent of a participant under the legal age of majority, as applicable.
